ABSTRACT

Record-breaking levels of asylum seeking by families with young children continue at the United States/Mexico border, particularly the Rio Grande Valley sector. In this Commentary, our aim is to increase awareness by providing child and adolescent mental health care providers with an update on current migrant conditions, bringing to light issues of family separation previously highlighted in the Journal.1 For context, our international group has collected data, via 3 large-scale studies funded by the National Institutes of Health, from more than 600 Latinx immigrants seeking asylum at the US/Mexico border in the last 4 years, during which levels of violence, climate disruption, and poverty in the Northern Triangle of Central America (ie, El Salvador, Guatemala, and Honduras) and Mexico have propelled high and sustained levels of asylum seeking in the United States. We contribute expertise in clinical psychology, Latinx psychology, attachment disruption, and public health, as well as this front-row perspective.

ABSTRACT

Porous Silicon (PSi) is an ideal material to build biosensors due to its large surface area and biocompatibility. However, it lacks of selectivity. By adhering bilayer lipids, active sites are added for vital biochemical processes. Such processes are promoted by different proteins, which aid to detect pollutants and drugs, among other. The present work is a systematic theoretical study at the density functional theory level on PSi models, functionalized with H and OH. Several concentrations of such functional groups were assessed at the pores to elucidate the reactivity via Fukui indexes of electrophilic and nucleophilic attack. The 1,2-dimyristoyl-sn-glycero-3-phosphocholine(DMPC) lipid was used as a probe system to interact with the PSi. The attraction was evaluated as electrostatic with a van der Waals contribution. The adsorption was highly selective to the degree of functionalization at the pore. The PSi facets (100) and (001) showed different mechanisms of interaction with the DMPC lipid. The theoretical absorption spectra addressed that the DMPC lipid could be identified with intensity variations coming from the degree of functionalization at the pore, which may be further rationalized experimentally. The present methodology may aid to tailor novel materials to capture and identify adverse agents present in the environment.

ABSTRACT

Objetivos: Determinar las características demográficas, clínicas y de manejo analgésico de una población con dolor oncológico remitida a nuestra unidad de dolor (UD). Descripción de su manejo clínico una vez recibidos en la unidad. Detectar aspectos de mejora.Material y métodos: Estudio retrospectivo y descriptivo, realizado durante un periodo de 23 meses, entre noviembre de 2019 y diciembre de 2021, de todos los pacientes oncológicos derivados a la UD, cuya causa de derivación fuera dolor de origen oncológico.Resultados: Se analizaron 78 pacientes, el 63,2 % hombres y el 46,8 % mujeres. La edad media poblacional fue de 64,84 ± 12,623 años. El 44,7 % fueron remitidos por servicios quirúrgicos. El 75 % presentaba dolor moderado o severo. Los tumores más frecuentes fueron los abdominales (31,6 %), y cabeza y cuello (22,4 %). La causa del dolor fue en 48,7 % de los casos de la infiltración tumoral y en un 60,5 % un dolor de origen de mixto. En el momento de la remisión el 60 % de los pacientes recibía opioides mayores, con una dosis de equivalentes diarios de morfina (EDM) de 163,57 ± 167,10 mg y el 38 % recibía antineuropáticos. El tiempo medio para atender a estos pacientes desde el momento de solicitarla fue de 9,18 ± 9,73 días. Se realizó intervencionismo menor en el 56,6 % (43) de los pacientes y mayor en el 2,6 % (2). Se inició en la UD el tratamiento con fármacos antineuropáticos en el 68,4 % de los casos. Tras el manejo en la UD, un 72,4 % de los pacientes refirieron mejoría del dolor.Conclusiones: Mejorar la tasa de remisión de pacientes desde servicios como Oncología médica y Atención primaria. Mejorar los tiempos de remisión a las unidades de dolor. Ajustar mejor los tratamientos analgésicos antes de la remisión. Generar un protocolo de remisión sencillo de pacientes que incluyan pautas básicas de manejo del dolor. Mejorar el diagnóstico de dolor neuropático. Aumentar la cartera de servicios de intervencionismos de la UD....(AU)


Aim: Studying the demographic profile, clinical characteristics and analgesic management of an oncologic population sent to our pain unit. To describe the pain management in our unit. To detect management aspects to be improved.Methodology: Retrospective and descriptive study, performed in a period of 23 months, between November 2019 and December 2021, of all patients sent to our pain unit for cancer pain management.Results: A total of 78 patients were analyzed, 63,2 % men and 46,8 % women. The average age was 64,84 ± 12,623 years. 44,7 % were sent by surgical services. In 75% the pain was moderate or severe. The main cancer location was abdominal (31,6 %), and head and neck (22,4 %). In 48,7 % the pain was originated by tumoral infiltration and in 60,5 % the pain was judged to be mixed. At the moment of the arrival 60 % of patients were on opioids, with an average dose of 163,57 ± 167,10 mg EDM and 38 % were on antineurophatic drugs. The average time to attend the patients from the moment or request was 9,18 ± 9,73 days. A minor interventional procedure was performed in 56,6 % (43) of the patients, and a major intervention in 2,6 % (2). We started antineurophatic drugs in 68,4 % of the cases. During the period of pain management in our unit a 72,4% of the patients referred an improvement of their cancer related pain.Conclusions: It is necessary to improve the rate of remission from medical oncology departments and primary care physicians. To reduce the remission time to our unit from the referral services. To improve analgesic management before referral. To create an easy protocol for remission of patients that includes basic pain management instructions. To improve the rate of neuropathic pain diagnosis before referral. To expand our interventional technics portfolio. Pain units can improve cancer related pain management. To create multidisciplinary cancer pain comities.(AU)

ABSTRACT

Desórdenes sistémicos de la homeostasis mineral o fallas en la mineralización de la matriz extracelular pueden afectar tanto a las piezas dentarias como a su aparato de sostén, conduciendo al fracaso de los tratamientos odontológicos. Existe poca evidencia científica sobre modelos animales que permitan es-tudiar la respuesta de tejidos dentarios/paradenta-rios frente a este tipo de desórdenes. El objetivo del presente trabajo fue estudiar experimentalmente al-teraciones de la biomineralización inducidas por bis-fosfonatos en ratas Wistar. Para ello, 20 ratas Wistar hembras y machos (35±10 g) fueron tratados con 20 mg/kg de etidronato bisódico (EHBP) o solución fi-siológica (control) vía intraperitoneal 5 veces por se-mana durante 3 semanas. Luego de la eutanasia, se extrajeron las mandíbulas para su procesamiento y análisis histológico (H&E). Mediante microfotografías digitales se evaluó: área ósea/área total, espesor de ligamento periodontal, áreas de dentina y de cemen-to. Los resultados se analizaron estadísticamente mediante el test T de Student para comparaciones entre grupos y el test ANOVA de 2 vías para las com-paraciones intersexo. Los grupos EHBP presentaron mayor proporción de osteoide, dentina y cemento no mineralizados respecto a los controles, siendo más marcadas las diferencias en las hembras. Las hem-bras del grupo EHBP mostraron una disminución significativa en el espesor del ligamento periodontal respecto de los controles, aunque esta tendencia no se observó en machos. Los resultados del presente estudio demuestran que el EHBP, en la dosis y tiempo estudiados, altera los procesos de biomineralización tanto del tejido óseo como de los tejidos dentarios (AU)


Systemic disorders of mineral homeostasis or alterations in the mineralization of the extracellular matrix can affect both dental and supportive tissues, leading to the failure of dental treatments. There is a lack of scientific information about animal models that allow to study the response of dental/paradental tissues in this type of disorders. The aim of the present work was to study the biomineralization alterations induced by bisphosphonates in Wistar rats and to study the response of dental and paradental tissues. Twenty Wistar rats (35±10 g) were divided in control (females, males) and EHBP (females, males) groups. The EHBP group received 20 mg/kg of ethidronate bisodium intraperitoneally 5 times a week for 3 weeks, while the control group received saline solution. After euthanasia, mandibles were resected and processed histologically to obtained oriented sections for H&E staining. Photomicrographs were used to evaluate: Bone area/total area, periodontal ligament thickness, dentin and cement area. Results were statistically analyzed using the Student's T test for comparisons between groups and the 2-way ANOVA test for male and female comparisons. The EHBP groups showed a higher amount of non-mineralized osteoid, dentin and cement compared to control groups, being more evident in females. Females in the EHBP group showed a significant decrease in periodontal ligament thickness compared to controls, although this profile was not observed in males. The results of the present study demonstrate that EHBP, at the dose and time studied, alters the biomineralization processes of both bone and dental tissues (AU)

ABSTRACT

Objetivo: Se presenta una revisión sistemática con el objetivo de determinar si los niños escolares con cardiopatías congénitas participan en intervenciones psicológicas en el proceso de rehabilitación cardiovascular. Método: Se hizo la revisión guiada por el método PRISMA en las bases de datos: Dialnet, Pub Med, Redalyc, Sciencie Direct, Scielo, Google Académico de artículos publicados en idioma inglés, portugués y en español. Los criterios de inclusión son: intervenciones de rehabilitación que contengan acciones para el manejo de factores psicológicos. Fueron evaluados para elegibilidad 23 informes, solo uno fue elegido a través del método de tabulación de datos. Resultados: Se evidenció que estos programas para niños cardiópatas son escasos, se centran en la actividad física, desestimando las variables sociopsicológicas. Se propone implementar acciones de intervención estandarizadas con los pacientes cardiovasculares congénitos y su familia desde la psicocardiología según sus necesidades.(AU)

Introducción: La reserva cognitiva es la capacidad para tolerar mejor el daño cerebral mediante recursos cognitivos preexistentes y compensatorios. Un método de evaluación es el cuestionario de reserva cognitiva (CRC) de Rami. El objetivo fue realizar un análisis de la calidad informativa del CRC desde la teoría de respuesta al ítem (TRI), con el fin de aportar datos más precisos sobre la fiabilidad de consistencia interna. Se probó, además, la validez convergente con medidas de atención, memoria de trabajo y fluidez. Sujetos y métodos: Participaron 210 argentinos de población general (media edad, 66,8 años). El CRC se administró junto con el test de dígitos y tres tareas de fluidez. Se ajustó un modelo de respuesta graduada desde la TRI con estimación de parámetros de discriminación (a) y dificultad (b), y se elaboró una curva de información del CRC. Se efectuaron correlaciones bivariadas y parciales. Resultados: La TRI indicó una alta discriminación para los ítems del CRC ‘Escolaridad’ y ‘Nivel de ocupación’ (tanto para la versión de ocho ítems como para la versión de seis ítems). En el CRC de ocho ítems se obtuvo una baja discriminación para ‘Formación musical’ y ‘Juegos intelectuales’. En ambas versiones del CRC, la curva indica mayor valor informacional a bajo nivel del constructo. Hubo correlación con el test de dígitos y con las tareas de fluidez, incluso al controlar por edad. Conclusiones: El presente estudio es el primer análisis del CRC desde la TRI, que concluye que el instrumento resulta más confiable cuando se aplica a sujetos con menor reserva. El CRC posee aceptable validez convergente.(AU)

ABSTRACT

Para medir el éxito de un deportista y su ejecución deportiva, se requiere cuantificar el máximo rendimiento que desempeñe durante los entrenamientos y campeonatos, a partir de un instrumento válido, fiable y adaptado al contexto peruano. Sin embargo, hasta el momento no existen evidencias psicométricas que respalden el uso de alguno para deportistas calificados. El objetivo principal del presente estudio consiste en evaluar las propiedades psicométricas del Inventario de Ejecución Deportiva (IPED) en la población mencionada. Método:Participaron 255 deportistas con edades entre 14 y 38 años (N: 23; DT: 4.29), pertenecientes a modalidades afiliadas al Instituto Peruano del Deporte (IPD), y que representan al país de manera activa. Para el recojo de información se utilizó una ficha sociodemográfica, el Inventario de Ejecución Deportiva (IPED y el Cuestionario de Fortaleza Mental (FM). Resultados: El modelo IPED, presentó un buen ajuste de datos (SBχ2= 2243.096 / df= 798; CFI= .955; SRMR= .095; RMSEA= .084 [IC90%: .080-.069] y una consistencia interna adecuada para sus 7 dimensiones, que oscila entre ω = .841 y el menor valor ω = .754. Asimismo, se observaron correlaciones positivas y estadísticamente significativas al vincularlo con las dos dimensiones del Cuestionario FM: Confianza (AC=.678; CAN= .557; CAT= .457; CVI= .328; NM= .425; CAP= .641; CAC= .662) y Control (AC= .558; CAN= .733; CAT= .682; CVI= .171; NM= .272; CAP= .432; CAC= .563). Conclusión: El IPED brinda evidencia de validez y confiabilidad adecuadas para medir el desempeño deportivo. (AU)


To measure athlete’s success and his sports performance, it is required to quantify the maximum performance developedduring seasons of trainings and championships; using a valid, reliable,and adapted to peruvian context instrument. However, caution is urged when considering there is no psychometric evidence to support the use of any scale related to peruvian high performance athletes’sample. The current study was designed to evaluate the psychometric properties of the Sports Performance Psychological Inventory (IPED) in the mentioned sample. Method: Participated 255 peruvian athletes with ages between 14 and 38 years (N:23; SD: 4.29); who actively represents the country and in sports modalities affiliated with the Peruvian Sports Institute (IPD). For the collection of information, a sociodemographic record, Sports Performance Psychological Inventory (IPED)and the Mental Toughness Questionnaire (MTQ) were used. Results: IPED model demonstrated, that the seven-dimensional model had an acceptable fit (SBχ2 = 2243.096 / df = 798; CFI= .955; SRMR= .095; RMSEA= .084 [IC90%: .080 -.069] and reliability analyses revealed high internal consistency (ω= .841 > ω= .754). In addition, scores correlated positively between IPP and MTQ dimensions (AC= .678; CAN= .557; CAT= .457 ; CVI= .328; NM= .425; CAP= .641; CAC= .662 ) and Control (AC= .558; CAN= .733; CAT= .682 ; CVI= .171; NM= .272; CAP= .432; CAC= .563) Conclusion: The IPED provides adequate evidence of validity and reliability to measuresports performance. Para medir o sucesso de um atleta e seu desempenho esportivo é necessário quantificar o desempenho máximo que ele realiza durante os treinos e campeonatos a partir de um instrumento válido e confiável adaptado ao contexto eduardo.manzanares@upc.pe peruano.No entanto, até o momento não há evidências psicométricas para apoiar o uso de qualquer instrumento por atletas qualificados. Nesse sentido, o objetivo principal deste estudo é avaliar as propriedades psicométricas do Inventário de Performance Psicológicapara o Esporte(IPED) na referida população. Método:participaram 255 atletas que representam ativamente o país nas modalidades esportivas filiadas ao Instituto Peruano de Esportes (IPD). As idades variaram de 14 a 38 anos (N: 23; DT: 4,29). Para a coletadas informações foi utilizado um inventáriosociodemográfico, o Sports Performance Inventory e o Mental Strength Questionnaire. Resultados:O modelo sete-dimensional IPED apresentou um bom ajuste dos dados (SBχ2 = 2243,096 / df= 798; CFI= ,955; SRMR= ,095; RMSEA= ,084 [IC90%: 0,080-0,069] e consistência interna adequada para as dimensões que variam entre ω= 0,841 e o menor valor ω= 0,754. Da mesma forma, foram observadas correlações positivas e estatisticamente significativas entre as 7 dimensões do IPEDcom as dimensões de confiança (AC = 0,678; CAN = .557; CAT = .457; CVI = .328; NM = .425; CAP = .641; CAC = .662) e Controle (AC = .558; CAN = .733; CAT = .682; CVI = .171; NM = .272; CAP = .432; CAC = .563) do Questionário FM.Conclusão:Conclui-se que oIPED apresenta evidências de confiabilidade e validade adequadas para medir o desempenho esportivo. Introducción: incrementar el consumo de frutas y verduras es una prioridad, objetivo fundamental en las políticas públicas a nivel mundial. Dado que dicho consumo en los escolares de Jalisco (México) se encuentra por debajo de las recomendaciones alimentarias, es esencial identificar los determinantes que influyen en su consumo de frutas y verduras, para promover el desarrollo de acciones que contribuyan a mejorarlo. Objetivo: identificar los determinantes socioeconómicos y sociodemográficos del consumo de frutas y verduras en las madres de familia y los hogares de escolares de Jalisco, México. Método: estudio transversal analítico, realizado durante el primer trimestre del año 2020 utilizando una frecuencia de consumo de alimentos validada y un cuestionario sobre factores sociodemográficos y socioeconómicos. Resultados: el bajo nivel educativo de las madres de los escolares se asoció con un menor consumo de verduras y el bajo nivel de ingresos en el hogar se asoció con un menor consumo de frutas por los escolares. Conclusiones: el bajo nivel educativo de las madres y el bajo nivel de ingresos de los hogares fueron determinantes asociados al consumo de frutas y verduras de los escolares. Sin embargo, se presentaron diferencias en cuanto a los determinantes para las frutas y para las verduras. Estos factores y sus diferencias deben de tenerse en cuenta para la planificación de acciones que contribuyan a mejorar la ingesta de frutas y verduras en la población escolar (AU)


Introduction: increasing fruit and vegetable consumption is a priority. It has been prioritized as a fundamental objective of public policies worldwide. Given that such consumption in schoolchildren in Jalisco (Mexico) is below the dietary recommendations it is crucial to identify the determinants that influence this consumption to promote the development of contextualized actions that improve it. Objective: to identify the socioeconomic and sociodemographic determinants of fruit and vegetable consumption among mothers and households of schoolchildren in Jalisco, Mexico. Method: an analytical, cross-sectional study carried out during the first quarter of 2020. A validated food consumption frequency and a questionnaire on sociodemographic and socioeconomic factors were used for its development. Results: a lower educational level of the mothers of schoolchildren was associated with a lower consumption of vegetables by schoolchildren. In turn, a lower household income level was associated with a lower consumption of fruits in schoolchildren. Conclusions: a low educational level of mothers and a low household income were determinants associated with fruit and vegetable consumption in schoolchildren. However, there were differences in the determinants for fruits and vegetables. It is essential to consider these factors and their differences in order to plan actions that contribute to improving fruit and vegetable intake in the school population (AU)

INTRODUCTION: Assisted living residents are a fragile population with limited access to health care. In recent years, telemedicine has developed in several specialties, including ophthalmology. The main objective of our study is the validation of an ophthalmology telemedicine protocol in assisted living facilities. MATERIALS AND METHODS: This is a retrospective, multicentric, descriptive study including 4 assisted living facilities in the Rennes area. An ophthalmic technician examined residents for one half-day per week. The examination was performed directly in the patient's room, in bed or in a chair, with portable equipment. An ophthalmologist from our service interpreted the results remotely on a deferred basis within 7 days. Appropriate medical or surgical care was then offered to the patient depending on the diagnosis. RESULTS: We included 113 residents. A complete, interpretable examination was achieved in 84.1% of cases. One or more ophthalmic conditions were diagnosed in 57.5% of the residents. Of the residents who were then seen at the university medical center, 65% underwent a scheduled surgical procedure or laser. Visual acuity was significantly improved subsequent to the telemedicine encounter. DISCUSSION: The success rate of a complete, interpretable examination allows us to now validate our nursing home examination procedure. In addition to bringing a large amount of initially unknown information to the medical and paramedical staff of the structure, ophthalmology telemedicine allows for a significant improvement in visual acuity. CONCLUSION: This project validated the feasibility of ophthalmology telemedicine in an assisted living setting. This protocol may also be applicable to other health care settings (penitentiaries, mental health institutions, etc.).

INTRODUCTION: Cognitive reserve is the brain's capacity to actively overcome damage by means of pre-existing cognitive resources and compensatory processes. This capacity is enhanced by education, occupational attainment, learning languages and the habit of reading, among other important factors. This study aimed to investigate the psychometric properties of Rami et al's Cognitive Reserve Questionnaire in an Argentinian population. MATERIALS AND METHODS: The sample that participated in the study comprised 203 Argentinian adult volunteers from the general population (mean age, 66.1; standard deviation, 8.1). The Cognitive Reserve Questionnaire was administered together with the Montreal Cognitive Assessment. Confirmatory factor analysis was carried out to test the unidimensionality of the questionnaire. In addition, reliability was estimated by the categorical omega coefficient and associative validity was analysed by means of bivariate correlations. RESULTS: Factor analysis confirms the unidimensionality of the questionnaire with good fit indices. Items 5 (musical training) and 8 (intellectual games), however, show low factor loadings. The test has acceptable reliability, with a categorical omega coefficient = 0.72, although it should be noted that the average variance extracted (0.41) is below the recommended criterion of 0.5. Good associative validity is observed: r (200) = 0.4; p < 0.001. CONCLUSIONS: The Cognitive Reserve Questionnaire is an instrument with acceptable psychometric properties for use in an Argentinian population. More research is required to further investigate the low factor loadings of items 5 and 8 and to strengthen the evidence of reliability.
